All the following forms must be completed, executed or electronically signed (Accepted) and submitted to Oakland County Cremation Services (OCCS). In reliance on your submission, OCCS/Gramer Funeral Home will obtain the remains of the Deceased and have the cremation performed on our and the crematory’s time schedule. (You may also print out the forms, complete them and deliver them to Gramer Funeral Home or fax them to 248-435-9009.)

Upon cremation, we will deliver the remains as you indicated on the Death Certificate Information Form or to the home address of the first named Authorized Representative on the OCCS Authorization.
